On the map 'Altar of the Athame' the final chest has a visual glitch where there is no animation i have experianced this 3 times in a row. not just once. Hope this can help!
Happens on Unholy Catacombs too
Quite often on Unholy people cant open chest because Buff Beams are not destroyed along the top of the wall also
It's happening on many different maps and modes.
